Occupational Therapist

This position is located in Far North Queensland (FNQ), Cairns, Australia.

We are looking for an Occupational Therapist (OT) who wants to work for a company that cares about their development and support as much as they care about their clients. You will be a qualified Occupational Therapist, registered with AHPRA to join our team to work with adult, adolescent and paediatric NDIS participants in the community in accordance with the goals, policies and requirements of Independent Support Solutions (ISS).

Key Responsibilities

  • Demonstrate competent knowledge, expertise and skill in the provision of Occupational Therapy services appropriate to the level of experience in accordance with prescribed professional and ethical standards, existing evidence-based practice and NDIS requirements. For complex or novel cases, you will seek direction from management as appropriate
  • Maintain appropriate documentation and patient records in line with ISS’s Policies and Procedures and contribute to the maintenance of accurate and timely information systems (statistics collection, quality data). Ensure that time spent across clinical and non-clinical work is in keeping with assigned requirements
  • Participate in quality and service improvement activities to continually improve patient/client care, including the development of clinical practices, procedures and protocols within the work environment
  • Develop evidence-based therapy plans and deliver therapy interventions to clients within their home/ community environment
  • Work closely with your interdisciplinary team to support clients to achieve their goals
  • Engage with internal mentoring and other support to grow your skill.

Support Coordinator

This position is located in Far North Queensland (FNQ), Cairns, Australia.

As a Support Coordinator and a key member of our growing and dynamic team, you will assist participants in the coordination and implementation of all supports through their NDIS funded plans.

You will regularly communicate with participants, their families and carers to best maximize the quality of support and value for money and build on your own knowledge for the best available services. You will also work as part of a team of Support Coordinators and be under the direction of a Team Leader.

You will be accountable for:

  • Completing support coordination within an agreed time frame;
  • Managing a portfolio of participants based on person-centred plans including ongoing tracking of billable hours;
  • Proactively taking part in participant review practices and coordinating a range of supports including informal, mainstream and funded;
  • Build high-quality professional relationships with internal and external stakeholders;
  • Develop, maintain and complete relevant documentation including service agreements;
  • Capture relevant data required by the NDIA;
  • Preparation of NDIA reports on outcomes, success indicators and financial tracking.

House Coordinator

This position is located in Far North Queensland (FNQ), Cairns, Australia.

The House Coordinator is responsible to lead a team of Disability Support Workers, managing and overseeing service delivery of each service outlet, and ensuring a happy, safe home for participants. The House Coordinator will also ensure professional standards, legislative requirements and that the organisation’s mission, values and objectives are met in accordance with the strategic plan.

The successful applicant will be a highly adaptable and active team player.

Skills and experience

  • The applicant will have experience working in a similar role, particularly knowledge of best practice procedures in a similar industry.
  • Effective communication skills and the ability to liaise effectively with staff and managers to coordinate shifts.
  • Well-developed capacity to manage the competing demands of a busy environment, work to deadlines and maintain attention to detail.
  • Previous experience with software such as CareMaster or similar.
  • Ability to work autonomously and as part of a team.

Duties include but are not limited to:

  • Managing a team of support workers within a service outlet and/or community setting.
  • Collaborating with participants and relevant stakeholders to gather input for developing person-centred care plans for ISS participants.
  • Collaborating with participants to identify goals through a person-centred approach and assisting to work towards achieving their individual goals.

SIL Support Worker

This position is located in Far North Queensland (FNQ), Cairns, Australia.

While supporting participants in a SIL environment supports workers will be required to:

  • Be available to work a 24hr roster in a SIL service (morning shifts, day shifts, sleepovers).
  • Be willing to cover 2 sleepover shifts per week.
  • Be able to confidently attend to personal care.
  • Have medication administration confidence.
  • Support and complete household chores and grocery shopping.
  • Have an understanding and experience with participants that have intellectual impairments and psychosocial disabilities.
  • The ability to think outside the box with creative ideas will enable participants to thrive, meet their goals, and achieve success.
  • The ability to work in a team environment.
